VNS Health Total is a health plan for people who have both Medicare Parts A and B and Medicaid and who need long-term-care services to remain safely at home. Eligibility is determined through a clinical assessment. You must be eligible for nursing home level of care as of the time of enrollment, based on the Community Health Assessment. Final plan enrollment is determined by New York Medicaid Choice.
