Keep Your Total Member ID Card Handy!
Keep your card handy, along with your Medicaid and Medicare cards. It’s the one card you’ll need when you go to the doctor, the dentist, or the pharmacy. Use it when you need to show a provider that you’re a member of CHOICE Total, to find your Member ID number, or to quickly find CHOICE contact information.
