Getting To and From Medical Appointments

Because seeing your doctors is so important to your health, we want to help make doing so as easy for you as possible. That’s why VNSNY CHOICE Total (HMO D-SNP) covers your transportation to and from all of your non-emergency medical appointments. In an emergency, always call 911.

Please note that we recently changed how to arrange for your rides. 

In the past, you would call CHOICE Member Services to reach our medical transportation service. We now ask that you call them directly.

This reduces the number of steps it takes to arrange a ride. It also shortens waiting times on the CHOICE phone line as fewer members use it to arrange their medical trips.

To Schedule Transportation

Please call VNSNY CHOICE Transportation Services, which are provided by Logisticare, at 1-877-718-4219  (TTY: 711), 8 am – 8 pm, Monday – Friday, and have the following information ready:  

  • Date of trip 
  • Pick-up time 
  • Medical office’s full street address and telephone number  
  • Any special needs (for example, if you use a wheelchair)

You must arrange your trip at least 48 hours (2 days) before your appointment. You can book up to 30 days ahead of time.

Returning Home

Because it’s hard to predict how long your doctor’s visit will last, you must wait until the visit is over to schedule your trip home. At that time, call our Transportation Service at 1-877-718-4219  (TTY: 711), 8 am – 8 pm, Monday – Friday, and a representative will arrange the return trip for you. Most likely it will be a different driver. Please expect to wait as your driver makes their way to the medical office.

IMPORTANT: If you have an emergency and need to visit the emergency room, always call 911.
