Keep Your Medicaid Coverage Active

If you don't get recertified by Medicaid, you can lose your coverage under CHOICE Medicare Preferred or Medicare Maximum 

Remember, every year you must re-apply for Medicaid coverage.

The Human Resources Administration (HRA) will send you an application form within 90 days of when your Medicaid coverage will end. It is very important that you complete the application process prior to your coverage ending.

  1. Review and complete the application form.
  2. Sign, date, and submit the application form along with any supporting document (proofs) to HRA.

If you have questions about recertification, please don't hesitate to call Member Services at 1-866-783-1444. Medicaid recertification specialists are available from 8 am – 8 pm, 7 days a week. 

Note: If you are also enrolled in CHOICE MLTC, Member Services will help you recertify your Medicaid coverage. If you are a CHOICE MLTC Member, watch for a letter from CHOICE when it's time to renew your Medicaid coverage.
