Keep Your Medicaid Coverage Active
IF YOU DON'T GET RECERTIFIED BY MEDICAID, YOU CAN LOSE YOUR COVERAGE UNDER SELECTHEALTH
Remember, every year you must re-apply for Medicaid coverage.
Medicaid will send you an application form within 90 days of when your Medicaid coverage will end. You will also receive a letter from SelectHealth with instructions for how to renew your Medicaid coverage. If you do not receive this information, please contact Member Services at 1-866-469-7774. It is very important that you complete this application process prior to your coverage ending.
- Review and complete the application form.
- Sign, date, and submit the application form along with any supporting document (proofs) to SelectHealth
