Transportation To and From Medical Appointments

Because seeing your doctors is so important to your health, CHOICE wants to make it as easy as possible. That's why one of your most important benefits is coverage for transportation to and from medical appointments.

Transportation services are handled by LogistiCare. To schedule travel to and from your medical appointments, you can call LogistiCare directly at 1-877-718-4220, (TTY: 711), 8 am – 8 pm, Monday – Friday.

How many transportation rides am I entitled to?
There is no limit to the number of trips you can take. 
 
How do I request transportation?
To schedule transportation to a medical appointment, contact LogistiCare at 1-877-718-4220 (TTY: 711) and have the information listed below ready.
 
You will need the following information when you call:
  • Member ID 
  • Date of trip and pick-up time 
  • Medical office’s full street address and telephone number  
  • Any special needs (for example, if you use a wheelchair)
Transportation must be scheduled at least 48 hours (2 days) before your appointment. You can book transportation up to 30 days in advance.
 
What locations are covered by my transportation benefit?
Transportation may be used for any non-emergency medical appointment, but NOT for other types of trips such as grocery shopping, hair appointments, or visits to family members.
 
If you have an emergency and need to visit the emergency room, you should always call 911.
 
How do I get home from my doctor's appointment?
  • Call LogistiCare to arrange for the return trip home.
  • Provide the transportation representative with a working telephone number. A provider will call when they are outside.
     
How do I confirm, cancel or change transportation I’ve already arranged?
Call LogistiCare at 1-877-718-4220, (TTY: 711) when you want to confirm, cancel or change transportation you’ve already arranged. Please try to call in cancellations or changes 24 hours in advance.
