My Interdisciplinary Team
One of the advantages of being a VNSNY CHOICE FIDA Complete (Medicare-Medicaid Plan) participant is that your care is coordinated by a group of healthcare professionals that you help choose. This group is called your Interdisciplinary Team (IDT).
You are an important part of your IDT, along with your doctor and your VNSNY CHOICE Care Manager. With your approval, your IDT can also include other healthcare and service providers you need for your care.
Your IDT works with you to develop and manage your personal care plan. Your team will also coordinate services for you. Some example are:
- Your IDT will make sure your providers know about all the medicines you take. This can reduce any side effects.
- Your IDT will help you schedule and get to appointments with doctors and other providers.
- Your IDT will make sure your test results are shared with all your doctors and other providers.
To reach your IDT, simply call Participant Services. If you have questions about coverage decisions or making an appeal, please call Participant Services to speak to your Care Manager or a member of your IDT Team. We’re here to help. VNSNY CHOICE FIDA Complete is a managed care plan that contracts with both Medicare and the New York State Department of Health (Medicaid) to provide benefits of both programs to Participants through the Fully Integrated Duals Advantage (FIDA) Demonstration.
