Transportation To and From Medical Appointments
Because seeing your doctors is so important to your health, CHOICE wants to make it as easy as possible. That's why one of your most important benefits is coverage for transportation to and from medical appointments.

How many transportation rides am I entitled to?
There is no limit to the number of trips you can take.
How do I request transportation?
To schedule transportation to a medical appointment, contact Members Services at 1-888-867-6555 and have the following information ready:
- Date of trip
- Pick-up time
- Medical office’s full street address and telephone number
- Any special needs (for example, if you use a wheelchair)
Transportation must be scheduled at least 48 hours (2 days) before your appointment. You can book transportation up to 30 days in advance.
If you have an emergency and need to visit the emergency room, always call 911.
What locations are covered by my transportation benefit?
Transportation may be used for any non-emergency medical appointment, but NOT for other types of trips such as grocery shopping, hair appointments, or visits to family members.
How do I get home from my doctor appointment?
Because it's hard to predict how long your doctor visit will last, you must wait until the visit is over to schedule your return home. At that time, contact Member Services and a representative will arrange for the return trip for you. It's not likely that the same driver who dropped you off will pick you up. And since traffic in New York is unpredictable, it's normal to expect some waiting time.
How do I confirm, cancel or change transportation I’ve already arranged?
Please contact Member Services and a representative will review the details of your trip with you. The representative will make any updates or cancel the trip as needed. To update or cancel a trip, please try to call 24 hours before the scheduled pick-up time.
