Transportation To and From Medical Appointments

As a member of VNSNY CHOICE Total (HMO D-SNP), you are entitled to an unlimited amount of trips to and from medical appointments. For more information, check your Evidence of Coverage or call Member Services.
 
Transportation services are handled by LogistiCare. To schedule travel to and from your medical appointments, call LogistiCare directly at 1-877-718-4219, (TTY: 711), 8 am – 8 pm, Monday – Friday.

How do I request transportation?
To schedule transportation to a medical appointment, contact LogistiCare at 1-877-718-4219, (TTY: 711) and have the information listed below ready.

You will need the following information when you call:

  • The Member ID number on your VNSNY CHOICE Total ID card
  • Date of trip and pick up time
  • Medical office’s full street address and telephone number
  • Any special needs (for example, if you use a wheelchair)
Transportation must be scheduled at least 48 hours (2 days) before your appointment. You can book transportation for up to 30 days in advance. 
 
What locations are covered by my transportation benefit?
Transportation may be used for non-emergency medical appointments only, and not for other types of trips such as grocery shopping, hair appointments, or visits to family members.
 
If you have an emergency and need to visit the emergency room, you should always call 911.
 
How do I get home from my doctor appointment?
  • Call LogistiCare to arrange for the return trip home.
  • Provide the transportation representative with a working telephone number. A provider will call when they are outside.
How do I confirm, cancel or change transportation I’ve already arranged?
Call LogistiCare at 1-877-718-4219, (TTY: 711) when you want to confirm, cancel or change transportation you’ve already arranged. Please try to call in cancellations or changes 24 hours in advance.
