Transportation To and From Both Medical and Non-Medical Appointments

If your VNSNY CHOICE plan includes a transportation benefit, the information below can help you make the most of it. VNSNY CHOICE FIDA Complete participants are entitled to an unlimited number of trips. For more information, check your Participant Handbook or call Participant Services.
 
How do I request transportation?
To schedule transportation to a medical appointment, contact Participant Services and have the following information ready:
  • Date of trip
  • Pick-up time
  • Medical office’s full street address and telephone number
  • The Participant ID number on your VNSNY CHOICE participant card
  • Any special needs (for example, if you use a wheelchair)
Transportation must be scheduled at least 48 hours (2 days) before your appointment. You can book transportation up to 30 days in advance. 
 
What locations are covered by my transportation benefit?
Transportation may be used for non-emergency medical appointments and non-medical events or services, such as religious services, community activities, or grocery shopping. 
 
If you have an emergency and need to visit the emergency room, you should always call 911.
 
How do I get home from my doctor appointment?
Because it's difficult to predict how long your doctor visit will last, you must wait until the visit is over to schedule your transportation home. At that time, contact Participant Services and a representative will arrange the return trip for you. It's not likely that the same driver who dropped you off will pick you up. And since traffic in New York is unpredictable, it's normal to expect some waiting time.
 
How do I confirm, cancel, or change transportation I’ve already arranged?
Please contact Participant Services and a representative will review the details of your trip with you. The representative will make any updates or cancel the trip as needed. To update or cancel a trip, we recommend that you call 24 hours before the scheduled pick-up time. You will be asked for the Participant ID number on your VNSNY CHOICE participant card.
 
If you need more details or need help with something else, please call Participant Services at 1-866-783-1444, 7 days a week, 8 am  8 pm (TTY for the hearing impaired: 711).

VNSNY CHOICE FIDA Complete is a managed care plan that contracts with both Medicare and the New York State Department of Health (Medicaid) to provide benefits of both programs to Participants through the Fully Integrated Duals Advantage (FIDA) Demonstration.

The State of New York has created a Participant Ombudsman program called the Independent Consumer Advocacy Network (ICAN) to provide Participants free, confidential assistance on any services offered by VNSNY CHOICE FIDA Complete. The Participant Ombudsman may be reached toll-free at 1-844-614-8800 or online at icannys.org. (TTY users call 711, then follow the prompts to dial 1-844-614-8800.)

Enrollment in VNSNY CHOICE FIDA Complete depends on contract renewal.
