To enroll in VNS Health MLTC you must be:

At least 18 years old

Eligible for Medicaid

In need of community-based long-term care (for a continuous period of 120 days) based on a functional assessment

In need of long-term care, for more than 120 days, based on a functional assessment (must be eligible for nursing home level of care, as of time of enrollment, based on the Community Health Assessment)

Able to remain safely at home with assistance for activities of daily living (such as bathing, dressing, walking, or preparing food)

Living in the service area of the plan* 

You or a member of your family can call us directly to discuss enrollment in the program. If you need help applying for Medicaid, please let us know.

*The VNS Health MLTC service area includes these counties: Albany, Bronx, Columbia, Delaware, Dutchess, Erie, Fulton, Greene, Herkimer, Kings (Brooklyn), Madison, Monroe, Montgomery, Nassau, New York (Manhattan), Oneida, Onondaga, Orange, Otsego, Putnam, Queens, Rensselaer, Richmond (Staten Island), Rockland, Saratoga, Schenectady, Schoharie, Suffolk, Sullivan, Ulster, Warren, Washington, and Westchester.
