1. Types of Information Collected
Personally Identifiable Information
Personally identifiable information ("PII") means information that specifically identifies you as an individual, such as your full name, telephone number, email address, postal address or certain account numbers. VNSNY CHOICE may ask you for certain kinds of PII to provide the various services available through the Site, such as enrolling in a VNSNY CHOICE health plan or accessing eligibility or claims information. You do not have to provide us with PII if you do not want to; however, that may limit your ability to use certain functions of the Site or to request certain services or information.
Non-Personally Identifiable Information
VNSNY CHOICE may also collect certain non-personally identifiable information ("Non-PII") from you as you use the Site, such as your Internet Protocol address, your browser type, your operating system, the pages you view on the Site, the pages you view immediately before and after you access the Site and the search terms you enter into the Site.
VNSNY CHOICE may also use "pixel tags" (sometimes called "web beacons" or "clear gifs"), which are tiny graphic images, on the Site. Pixel tags may help VNSNY CHOICE analyze users' online behavior and measure the effectiveness of the Site. 4. Security of Information
VNSNY CHOICE uses reasonable physical, electronic and administrative safeguards to maintain the security of PII it collects, including restricting physical access to our offices and computer servers, as well as employing electronic security systems and password protections that guard against unauthorized access. However, the transmission of data over the Internet cannot be guaranteed to be 100% secure. While VNSNY CHOICE will use reasonable means to protect the security of information you transmit to VNSNY CHOICE, VNSNY CHOICE cannot guarantee that such information will not be intercepted by third parties and VNSNY CHOICE will not be liable for the acts if third parties.
Additionally, please note that email is not encrypted and is not considered to be a secure means of transmitting information, so please do not email VNSNY CHOICE any confidential, financial or sensitive information.
