VNS Health Total Eligibility
You may be eligible for VNS Health Total (HMO D-SNP) if you are:
At least 18 years old
Eligible for Medicaid
Eligible for Medicare Part A and Part B
In need of long-term care, for more than 120 days, based on a functional assessment (must be eligible for nursing home level of care, as of time of enrollment, based on the Community Health Assessment).
Able to live safely at home with assistance for activities of daily living, such as bathing, dressing, walking, and preparing food.
Living in the VNS Health Total service area. This service area includes the following counties: Bronx, Kings (Brooklyn), Nassau, New York (Manhattan), Queens, Richmond (Staten Island), Suffolk, Westchester, Albany, Rensselaer, and Schenectady.
